OBJECTIVES"},"content":{"rendered":"<p><strong>View this as a PDF file: <a href=\"http:\/\/edspace.american.edu\/laurahorvat\/wp-content\/uploads\/sites\/1754\/2020\/12\/START-Now-Mission-Goals-and-Objectives.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">START Now Mission, Goals, and Objectives<\/a><\/strong><\/p>\n<h4><strong>OUR MISSION<\/strong><\/h4>\n<p>START Now aims to improve the health of the Westmoreland County community by providing school-based support for the prevention of adolescent overweight and obesity.<\/p>\n<h4><strong>PROGRAM GOALS<\/strong><\/h4>\n<ul>\n<li>Increase healthy dietary behaviors in Westmoreland County high school students.<\/li>\n<li>Increase physical activity and reduce sedentary behaviors in Westmoreland County high school students.<\/li>\n<li>Reduce the prevalence of overweight and obesity among Westmoreland County residents.<\/li>\n<\/ul>\n<h4><strong>OBJECTIVES<\/strong><\/h4>\n<h5>Process Objectives<\/h5>\n<ul>\n<li>During the START Now pilot, a program facilitator will deliver 120 classroom-based nutrition education lessons (12 lessons\/class x 5 classes\/semester x 2 semesters) to PTHS students.<\/li>\n<li>During the START Now pilot, 300 PTHS students will participate in a 12-lesson, classroom-based nutrition education curriculum.<\/li>\n<li>During the START Now pilot, a program facilitator will deliver 12 face-to-face nutrition education sessions (2 sessions x 3 time slot choices\/session x 2 semesters) to parents.<\/li>\n<li>During the START Now pilot, a parent\/guardian of at least 75% of students in the program will participate in two face-to-face nutrition education sessions for parents.<\/li>\n<li>During the START Now pilot, program staff will deliver 1,200 digital newsletters (4 newsletters\/parent x 300 parents) to the parents of program participants.<\/li>\n<li>During the START Now pilot, parents will open and engage with at least 60% of the program-specific digital newsletters that are delivered to them.<\/li>\n<li>During the START Now pilot, at least 75% of student participants will rate their experience in the program as \u201cgood\u201d or better.<\/li>\n<li>During the START Now pilot, at least 75% of parent participants will rate their experience in the in-person sessions as \u201cgood\u201d or better.<\/li>\n<\/ul>\n<h5>Impact Objectives<\/h5>\n<ul>\n<li>Six months after program completion, at least 50% of student participants will report improved food and meal choice behaviors vs. pre-program baseline through an adapted version of the School-Based Nutrition Monitoring secondary level student questionnaire (Hoelscher et al., 2003).<\/li>\n<li>Six months after program completion, at least 50% of student participants will report increased physical activity vs. pre-program baseline through an adapted version of the School-Based Nutrition Monitoring secondary level student questionnaire (Hoelscher et al., 2003).<\/li>\n<li>Upon program completion, at least 75% of parent participants will report having implemented at least two healthful changes (diet or PA-related) in their own home.<\/li>\n<li>By the end of the program, at least two changes per school will be implemented that improve the healthfulness of the school food environment (food offerings, behavioral economics, and\/or school food policy).<\/li>\n<li>By the end of the program, at least five student participants will form a program-sponsored, student-led coalition\/club that will advocate for continued improvements to the healthfulness of the school food environment.<\/li>\n<li>During its 2022 Wellness Policy Assessment, PTHS will improve to a \u201cfully in place\u201d rating for the adequate training of staff who carry out classroom-based nutrition education.<\/li>\n<li>By the end of the program, at least 75% of student participants will be able to propose appropriate changes to their own eating behaviors to meet the MyPlate Dietary Guidelines for adolescents of their age and sex.<\/li>\n<li>By the end of the program, at least 90% of student participants will be able to explain the PA and screen time recommendations for teens.<\/li>\n<li>By the end of the program, at least 75% of student participants will be able to create a healthy, balanced meal using foods sold in the school cafeteria.<\/li>\n<\/ul>\n<h5>Outcome Objectives<\/h5>\n<ul>\n<li>By the year 2032, the prevalence of obesity among Westmoreland County adults aged 18+ will be reduced to 28.8% or less.\n<ul>\n<li>Target-setting method: 10% improvement vs. 2017-2019 data (Pennsylvania Department of Health, 2020).<\/li>\n<\/ul>\n<\/li>\n<li>By the year 2032, the prevalence of overweight (inclusive of obesity) among Westmoreland County adults aged 18+ will be reduced to 60.3% or less.\n<ul>\n<li>Target-setting method: 10% improvement vs. 2017-2019 data (Pennsylvania Department of Health, 2020).<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>References<\/strong><\/p>\n<p>Hoelscher, D. Overweight and obesity are associated with serious short- and long-term health consequences, from psychosocial issues to increased risk of chronic disease, including type 2 diabetes, heart disease, stroke, and certain types of cancer (CDC, 2020b). Many of these diseases are leading causes of preventable, premature death (CDC, 2020b). Obesity and overweight also have a staggering economic cost. The healthcare costs of obesity are estimated at $147 billion per year in the U.S. \u2013 that\u2019s nearly $4,700 per second (CDC, 2020b)!<\/p>\n<p>The prevalence of childhood obesity is particularly alarming, as overweight and obese youth are more likely to become obese adults, to have difficulty losing weight in adulthood, and to have children who are also obese (Pandita et al., 2016). Youth obesity is most prevalent among adolescents aged 12 to 19, and thus, Healthy People 2020 objectives include reducing obesity and preventing inappropriate weight gain among this age group (Ruiz et al., 2019; Office of Disease Prevention and Health Promotion [ODPHP], 2020). Adolescence is also a critical developmental period during which autonomy develops and lasting health behaviors may be formed (Brown et al., 2017; Flodgren et al., 2010). Research shows that preventive interventions that occur during this \u201cwindow of opportunity\u201d may be particularly effective in positively shaping long-term health behaviors (Flodgren et al., 2010).<\/p>\n<h5><strong>The Local Issue<\/strong><\/h5>\n<p>Locally, overweight and obesity are serious health threats facing the residents of Westmoreland County, Pennsylvania (PA). Reducing obesity is one of the state\u2019s top five health priorities, but there is significant work to be done (LiveHealthyPA, n.d.). Currently, only 31% (\u00b12) of PA adults are at a healthy weight, and state-level Healthy People 2020 goals for reducing adult and adolescent obesity have not been met (Pennsylvania Department of Health [PDH], 2019). The economic burden of obesity in PA is enormous, with annual healthcare costs estimated at $5.3 billion; on a per resident basis, these costs top those of 38 other states (Wang et al., 2015). In Westmoreland County, 67% of adults are overweight or obese, and the leading causes of death (heart disease and cancer) are associated with these conditions (PDH, 2020). Further, 30% of PA high school students are overweight or obese and, between 2009 and 2019, the prevalence of obesity among white PA high schoolers rose from 10.5% to 14.8% (CDC, 2019a). This is of high concern, given the demographics of Westmoreland County (95% white) and the risks that adolescent overweight and obesity pose now and in adulthood (USCB, 2019b).<\/p>\n<h5><strong>Proposed Solution: START Now<\/strong><\/h5>\n<p>To address overweight and obesity in Westmoreland County, it\u2019s recommended that the Pennsylvania Department of Health fund the development of <strong>S<\/strong>upporting <strong>T<\/strong>eens for <strong>A<\/strong> healthie<strong>R<\/strong> <strong>T<\/strong>omorrow (START) Now. START Now is a school-based program for adolescents aged 14 to 18 that\u2019s aimed at the primary prevention of overweight and obesity. To confirm best practices and prove program value, a 1-year pilot at Penn-Trafford High School (PTHS) is recommended. With nearly 1,300 students, PTHS plays a central role in local teens\u2019 lives, and evidence shows that school can be an effective environment in which to promote health behavior changes (CDC, 2018). In total, 300 students are expected to participate in the PTHS pilot. Consistent with CDC (2018) recommendations, teens won\u2019t be selected for START Now based on BMI or weight status, and physical appearance will not be emphasized. Rather, the program will be anchored in the classroom during regular health courses, and will promote the development and maintenance of healthy dietary and physical activity habits in all PTHS students (CDC, 2018).<\/p>\n<p>START Now is a multi-component program that will increase students\u2019 knowledge and skills around healthy eating and adequate physical activity, influence the home environment through parent-targeted education, and increase access to healthful foods at PTHS. The program\u2019s primary component will be aimed directly at teens, rather than relying on knowledge transfer through parents or others; evidence shows that this is important due to adolescents\u2019 growing autonomy and intrinsic motivation (Weihrauch-Bl\u00fcher et al., 2018). However, parental involvement is also associated with lower obesity rates, so START Now will provide separate nutrition education to PTHS parents and guardians (Gray et al., 2019). In addition, START Now will aim to improve the healthfulness of the school food environment, as this has been associated with lower youth obesity rates (Gray et al., 2019). Following evaluation of the pilot, START Now may be adapted and extended to other Westmoreland County high schools.<\/p>\n<h5><strong>Benefits to Pennsylvania<\/strong><\/h5>\n<p>START Now can benefit the state of Pennsylvania by helping to reduce the significant human and economic costs of overweight and obesity in Westmoreland County, both now and in the future. Said another way, preventing adolescent overweight and obesity also addresses <em>adult <\/em>obesity and its consequences, helping to alleviate the state\u2019s $5.3 billion burden (Pandita et al., 2016). Although overweight and obesity are complex issues with a variety of behavioral, environmental, and genetic determinants, evidence shows that healthy dietary behaviors and adequate physical activity are crucial to prevent and manage these conditions, and that adolescence is a critical period in which to instill healthy habits (CDC, 2018; Flodgren et al., 2010). Evidence also shows that school-based obesity prevention programs can be not just cost-effective, but cost-saving (CDC, 2018). Investing in START Now today will benefit the state of Pennsylvania for years to come.<\/p>\n<h5><strong>Recipe for Success<\/strong><\/h5>\n<p>START Now has strong potential for success at PTHS and in greater Westmoreland County. First, the program speaks directly to adolescents, rather than relying on knowledge transfer through others; evidence shows that this is important for success (Weihrauch-Bl\u00fcher et al., 2018). Second, START Now uses a multi-component approach that addresses both dietary habits and physical activity &#8211; a best practice in youth obesity prevention (Psaltopoulou et al., 2019). And finally, the program promotes a more healthful school food environment, which has been linked to lower obesity rates (Gray et al., 2019). The START Now pilot program will be implemented at Penn-Trafford High School (PTHS) during the August 2021 \u2013 May 2022 school year. Following pilot program evaluation, START Now may be adapted and extended to other Westmoreland County high schools.<\/p>\n<h5><strong>Theoretical Basis<\/strong><\/h5>\n<p>The START Now program is rooted in Social Cognitive Theory (SCT) and is designed to facilitate behavior change by targeting the individual, interpersonal, and environmental levels of influence per the social ecological model (Contento, 2016). Thus, START Now has three key components: a nutrition education curriculum designed to increase teens\u2019 knowledge, skills, and self-efficacy around healthy eating and adequate physical activity, a nutrition education component for parents, and a component focused on improving the healthfulness of the school food environment.<\/p>\n<h5><strong>Student Nutrition Education<\/strong><\/h5>\n<p>Individually, adolescents require functional knowledge and skills to develop healthy habits, along with the self-efficacy to do so (Contento, 2016). START Now will speak to students directly through its nutrition education curriculum, rather than relying on knowledge transfer through parents or others; this is important due to teens\u2019 growing autonomy and intrinsic motivation (Weihrauch-Bl\u00fcher et al., 2018). The START Now student curriculum includes 12 face-to-face, 50-minute lessons across four modules focused on healthful eating and increased physical activity. The modules (Nutrition Fundamentals, Energy Balance, Creating Healthy Meals, and Real Life Choices) begin with basic knowledge and skills and progress to more complex practice and application. The interactive curriculum will be delivered by trained START Now program facilitators during PTHS health classes over a four-week period, with three lessons taught per week. Consistent with SCT, program facilitators will promote personal agency, independent decision-making, and critical thinking skills among student participants, and students will be provided with opportunities for both observational and hands-on learning (Contento, 2016). As part of the curriculum, students will also participate in a Healthy Habits Challenge, which will require them to set individual SMART goals, track their progress, and offer social support to one another (Contento, 2016). In total, 120 classroom-based lessons will be delivered to 300 PTHS students during the 2021-2022 school year.<\/p>\n<h5><strong>Parent Nutrition Education <\/strong><\/h5>\n<p>Interpersonally, teens remain influenced by their family environments and often make dietary choices based on what\u2019s available at home. Thus, START Now will also provide adult-directed nutrition education to encourage the parents of student participants to become actively involved in the program. The parent component will include face-to-face educational sessions and digital newsletters, delivered via email or text. During the PTHS pilot, program staff will deliver 12 adult nutrition education sessions (two sessions per parent) and 1,200 weekly newsletters (four per parent). The 75-minute educational sessions will be held on school grounds outside of regular school hours, and parents will gain knowledge and skills to help their teen(s) eat healthfully and increase physical activity. Ultimately, parents should apply these learnings by making healthful changes at home.<\/p>\n<h5><strong>Environmental Change<\/strong><\/h5>\n<p>At the environmental level, START Now aims to improve teens\u2019 access to healthy food choices \u00a0at school. The START Now program team will partner with school staff, administrators, food service, and students to identify opportunities to improve the school\u2019s food environment. These may include improved food offerings, application of behavioral economics in the cafeteria, and\/or changes to school food policy. As result of START Now, at least two such changes will be implemented. Additionally, START Now will sponsor the formation of a student-led coalition (one per school) that will advocate for further improvements to the school food environment; the coalition may also help to reshape social norms.
